Applicant Requirements:

• Applicant must be in active treatment, as defined by Pink Fund, for their breast cancer diagnosis throughout any funding.
• Applicant or spouse/partner must have been working at time of diagnosis, and are able to show a loss of working income due to applicant’s breast cancer diagnosis. (e.g., leave of absence, reduced hours, etc.)
• Applicant household income must not exceed 300% of the federal poverty level based on last year’s federal tax return.
• Applicant must have been diagnosed with breast cancer within 2 years of Pink Fund receiving the application.
